Clinical Pharmacy Specialist - Clinical Pharmacy Specialist Community Living Center/Palliative Care
The CLC/ PC Pharmacist (CPS) initiates, modifies, or discontinues medication therapy as a consultant for medication therapy management; designing, implementing, assessing, monitoring and documenting therapeutic plans using effective medication treatments. The CPS interacts with patients and interdisciplinary teams; performs assessments and orders tests to determine medication efficacy and toxicity. The CPS works under the Chief of Pharmacy and the Associate Chief of Clinical Pharmacy Services. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
