Clinical Pharmacy Practitioner (Home Based Primary Care)
The CPP functions as part of the HBPC care teams and is most effective when assigned to patients to support continuity of care. Care will be provided through a variety of modalities including, but not limited to, telephone, virtual care, chart consults and in-home visits. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
