Outpatient & Inpatient Physical Therapist (Full Time)
Job Summary: The Physical Therapist (PT) is responsible for performing an age and diagnosis specific assessment designed to gather date relevant to the patient's/residents/client's individual needs. This includes the evaluation and assessment of cognitive, sensory, neuromuscular, physiological, psychosocial and functional status. The PT develops a plan of care, initiates, performs and manages treatment as indicated, assesses response to treatment, and assists with discharge planning.
