Our Care Manager Coordinators (CMC) provide support and assistance to Care Managers to advocate for members who cannot advocate for themselves. They are responsible for coordinating, scheduling and promoting all case management activities with members for on-going evaluation of the medical and psychosocial needs of the member to facilitate the memberâ™s overall wellness.
â¢ Collaborates with Care Manager to develop a proactive plan of care to address identified issues to enhance the short and long-term outcomes as well as opportunities to enhance a memberâ™s overall wellness. â¢ Identify community resources to meet the memberâ™s need and benefits identified during assessment. â¢ Assists with implementation of strategies designed to reduce risk factors and barriers and address complex health and social indicators which impact care planning â¢ Conducts review of documentation from various sources, such as claims, to develop or identify potential issues. â¢ Uses a holistic approach to assess the need for a referral to clinical resources and other interdisciplinary team members. â¢ Collaborates with Care Manager and other key stakeholders in the memberâ™s healthcare in overcoming barriers in meeting goals and objectives, presents cases at interdisciplinary case conferences as necessary â¢ Understands and follows case management processes in compliance with regulatory and company policies and procedures under the guidance of the Case Manager. Utilizes motivational interviewing skills to ensure maximum member engagement and discern their health status and health needs based on key questions and conversation.
Required Qualifications -Minimum 3-5+ yearsâ™ experience in a clinical setting or practice -LBSW or LLMSW licensure -Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually -Excellent analytical and problem-solving skills -Effective communications, organizational, and interpersonal skills. -Ability to work independently -Effective computer skills including navigating multiple systems and keyboarding -Demonstrates proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, as well as some special proprietary applications
Preferred Qualifications -Coordination of Care experience, medical and/or behavioral care preferred
Education -Bachelorâ™s or Master's degree in Social Work -Certified Case Manager or Care Management experience is preferred
