The need for complex care associated with pharmacy agents continues to grow. Medication needs in community, discharge, and oncology settings continue for patients within University of Michigan Hospitals and Health Centers. This position supports community pharmacy as well as specialty pharmacy, University of Michigan Cancer Center Pharmacy, East Ann Arbor Pharmacy, and Brighton Center for Specialty Care Pharmacy.
Patients require management of complex disease states and medications. This often results in additional clinical care and proactive patient intervention. Pharmacists will provide care over a variety of disease states and assist in the development and implementation of clinical protocols, such as Medication Therapy Management (MTM) and immunization administration for these patients.
Pharmacists will rotate coverage between community pharmacy practice sites, including East Ann Arbor Pharmacy, Taubman Center Pharmacy (which serves primarily as a discharge pharmacy), Cancer Center Pharmacy, and Brighton Center Pharmacy. Ideal candidate must be flexible and adaptable to change as we are growing and the working environment will continue to change.
Pharmacists serve as preceptors for University of Michigan Pharm.D. students on a variety of rotations including medication reconciliation, direct patient care Introductory Pharmacy Practice Experiences (IPPEs) during the second and third year, and institutional unit-based hospital practice Advanced Pharmacy Practice Experiences (APPE) during the fourth year. In addition, pharmacists will assist with precepting pharmacy residents, including the PGY-1 Community Pharmacy Resident, as well as the PGY-2 Ambulatory Care Pharmacy Residents.
Order Verification/ Profile Review:
Pharmacists review, clarify, and process medication orders in a timely manner while assuring appropriateness of therapy. Mediation reconciliation is performed on each set of prescription orders assuring the smooth transition of care to the outpatient community setting.
Pharmacists provide input and suggestions for process improvements to improve the care that is provided to our patients. Activities include researching opportunities and initiating action plans with follow through until successful implementation of the improvement.
Supervision of Work Area:
Pharmacists provide functional supervision of technical staff working in their area including completion of job assignments and assuring adherence to institutional policies and procedures.
Doctor of Pharmacy Degree (Pharm.D.) from an accredited College of Pharmacy or BSPharm with commensurate experience.
Current license to practice pharmacy in the State of Michigan (including a Controlled Substance license), verifiable through the State of Michigan Department of Licensing and Regulatory Affairs, or eligibility to become licensed within 90 days of employment start date.
Ability to perform a variety of manual dexterity movements, including keyboarding and non-sterile and sterile product preparation. Fluency in reading, writing and speaking English. The job involves considerable standing and walking, as well as extensive work on a computer, along with fatiguing physical motions.
Completion of a PGY1 residency and/or BCPS certification
Prior retail or community experience is desired, along with prescription insurance knowledge and familiarity
Specialty, MTM, and/or immunization experience is desired.
Pharmacist, Pharm.D. is preferred.
The schedule for this position is day shift, working every seventh Saturday with occasional Sundays and holiday coverage.
This position may be underfilled at a lower classification depending on the qualifications of the selected candidate.
