![]() Behavioral Health Licensed Social Worker, Per Diem - Primary Care Network Department Description The Auburn and Roseville Clinics are a part of the UC Davis Primary Care Network servicing clients in the Sacramento area with a wide range of healthcare needs including family medicine, internal medicine/general medicine, and pediatric specialists. Job Summary THIS IS NOT AN H1- B OPPORTUNITY Responsibilities The Primary Care Behavioral Health Licensed Social Worker position is located at Network clinic locations which have consulting Psychiatrists available to support the LCSW and primary care in a collaborative care delivery model. The primary role of the LCSW is to assist patients and families/caregivers to cope with the social/emotional issues and practical arrangements related to the patient's illness, as well as promoting the delivery of effective, cost-efficient, quality patient care through the delivery of psychotherapy services aimed at treating mild to moderate depression. The LCSW delivers age-appropriate social work care to patients and their caregivers in accordance with agency policy and procedure and state and federal regulations. The LCSW serves as an integral member of the healthcare team providing behavioral health assessments, psychotherapy, and facilitating connection with other social work resources for coordination, treatment planning, information and referral to community resources and other social work services to meet the complex needs of patients and families in the hospital and clinic settings. The LCSW shall adhere to and promote the ethics and values of the social work profession, using the NASW Code of Ethics as a guide to ethical decision making. The LCSW will advocate for the patient's right to self-determination, confidentiality, access to supportive services and resources, and appropriate inclusion in decision making that affects their health and well-being. The LCSW will be an active participant and work collaboratively with the multidisciplinary team by recognizing and offering services that address the biopsychosocial needs of the patient The LCSW position requires a high level of clinical judgement, problem-solving skills, creativity, organizational and priority setting abilities, with excellent communication skills. The LCSW demonstrates the ability to fulfill these responsibilities through clinical assessment, accurate and sound judgement, and maintaining professional communication. The LCSW may be asked to participate in special projects and collect data to improve the quality of services delivered to our patients. The LCSW must be flexible as changes in technology, healthcare coverage, regulatory requirements, and our patient population may mandate changes to workflow, work standards, and work practices on an as needed basis.
