As a Director Care Management you need to know how to:
Promote mission, vision and values Good Samaritan Medical Center and Sisters of Charity Leavenworth Healthcare.
Actively fulfill Best in the Nation goals.
Hold responsibility for human resource functions: interviewing, selection, orientation, education/training, discipline, termination for this department.
Handle fiscal management functions: develop/monitor budget, cost center management analysis.
Strategically plan, goal set, organize/evaluate assigned sections of the department.
Maintain knowledge of JCAHO/State/Federal and other regulatory agencies regarding discharge planning, conditions of participation and care management.
Maintain knowledge of performance improvement programs for care management.
Develop and monitor policies, procedures and productivity standards.
Maintain knowledge of applicable software applications.
Maintain knowledge of reimbursement mechanisms, providers, regulatory issues pertaining to re-imbursement and Utilization Management.

RN, MSW or related field and experience in Care Management, required
Minimum of three (3) years of Care Management experience, required
Working knowledge of Managed Care concepts, required
Supervisory experience, required
Knowledge of Medicare Conditions of Participation, required
