Within the Capacity Management division, the Patient Flow Coordinators (PFC), coordinate and optimize patient flow across Inpatient Units/ED/PACU/OBS/OR of University Hospital, Mott Hospital and Frankel Cardiovascular Center to promote quality, service excellence, patient and family centered care, and maximize throughput. Own end-to-end patient flow process as well as clinical and operational issues surrounding patient placement in partnership with medical and nursing staff. Provide ongoing updates of anticipated and confirmed discharges and transfers. Collaborate with ancillary services and disciplines to eliminate barriers to timely transfers and discharges to optimize bed utilization. Functions as the liaison and works in collaboration with a multi-disciplinary team and variety of internal and external resources to meet patient flow objectives and resolve issues.
Duties include but are not limited to:
Provide excellent customer service to internal and external customers.
Provide effective communication with team members and other departments to ensure seamless process.
Provide accurate and timely follow-up with staff, patients and families.
Active problem-solving of issues related to patient flow.
Communicate status of house to key stakeholders (units, ED, PACU, leadership, etc.).
Patient Placement Planning and Coordination:
Serve as the primary, centralized owner of patient flow activities.
Maintain current knowledge of specific unit patient placement criteria.
Round continuously on all patient care areas to gather and report real time activity as well as actively problem-solving and identifying potential issues/barriers/bottlenecks to patient flow.
Provides ongoing updates of anticipated and confirmed discharges and transfers. Collaborates with ancillary services and disciplines to eliminate barriers to timely transfers and discharges to optimize bed utilization. Communicates and assists with priority discharges.
Collaborate with nursing and medical staff to assess levels of care required for current patients in ICU/moderate care/telemetry. Identify options and facilitate transfers to higher level of care when needed.
Facilitate intra-unit transfers to optimize medically necessary private, cohort rooms and patient satisfaction.
Provide ongoing assessment and communication of bed availability and changes in patients’ clinical condition to ABCC.
Assess patient volumes and workload intensity in all areas for coordination and appropriate patient placement.
Activate and lead the hospital’s high occupancy plan based on guidelines established and work with cross-functional team to initiate/activate/manage/close identified overflow patient care areas.
Partner with charge nurses to identify and problem-solve patient placement barriers to expedite bed assignment process. Build relationships between ABCC and stakeholders.
Monitor and communicates issues related to empty bed time (blocked beds) and nurse staffing on capacity.
Prioritize patient placement, providing direction to the Admission Triage Associates, from multiple in-coming locations i.e. home, clinic, outside hospital transfers, PACU, and emergency department from all hospitals.
Develop a plan for placement of next day scheduled admissions in collaboration with the patient care areas and the Admission Triage Associates.
Develop a plan for placement of outside hospital transfers in collaboration with Admission Triage Coordinators based on bed availability and medical priority.
Provide cross coverage to the Admission Triage Coordinator as needed.
Manage placements escalated by Admission Triage Associate.
Ensure pending and confirmed discharges are entered into system.
Facilitate communication of area admissions and bed availability through daily structured processes such as bed briefing or others as determined by the ABCC.
Provide status report and ongoing issue update at shift change.
Manage multiple competing tasks and deadlines with energy and efficiency.
Serve as a resource regarding institutional and departmental policies and procedures.
Complete other duties as assigned.
Bachelor’s degree in nursing or a related allied health discipline or equivalent combination of education and experience is preferred.
Minimum of 3-5 years of clinical experience. Critical or emergent care experience is preferred.
Excellent clinical knowledge and assessment skills to evaluate and prioritize patient flow issues such as identifying medical and discharge needs of patients and families.
Demonstrated strong interpersonal skills with the ability to interact with the healthcare team, peers, patients, families and community to work as a team, facilitate problem resolution and make difficult decisions under pressure.
Demonstrated ability to plan, organize and direct the activities of others
Demonstrated excellent problem solving, conflict resolution, verbal and written communication skills, and customer service skills.

Night Shift, 6p-6a; Days of the week will vary
