PositionSummary:The Advanced Clinical Pharmacist isresponsible for providing all levels of pharmacy clinical services. Thisincludes monitoring patient medication regimens and providing appropriateinterventions as well as supporting all aspects of pharmacy operations.

Positionduties (included butnot limited to):
·Interpret orders and transcribes or verifiesthem accurately and efficiently into the pharmacy computer system.
·Accurately compound and dispensepharmaceuticals.
·Complieswith facility policy and procedures and regulatory requirements.
·Monitor drug therapy regimens forcontraindications, drug-drug interactions, drug-food interactions, allergiesand appropriateness of drug and dose.
·Provideclinical consultation and clarification to practitioners.Suggest appropriate, cost-effectivetherapeutic alternatives to medical staff as needed.
·Sustain the hospital formulary, minimizingnon-formulary procurements, utilizing therapeutic interchanges and promotesrational drug therapy selection.
·Attendand actively participate in daily multidisciplinary and bedside rounds.
·Support the hospital formulary by working withmedical staff to promote rational drug therapy and make improvements identifiedby drug utilization evaluations.
·Provide accurate and timely medicationinformation to healthcare staff and patients.
·Actively participate in pharmacy clinicalservices such as drug consults, nutrition support, pharmacokinetics,anticoagulant monitoring, pain management, IV to PO, renal dosing and otherprograms as defined by Pharmacy Leadership.
·Participatein the antimicrobial management and other clinical programs.
·Document all clinical activities andinterventions accurately and completely.
·Responsible for mentoring and education aswell as ongoing analysis of interventions to ensure accurate and appropriatedocumentation and provide summary reports (if assigned).
·Supportand participate in the goals of the organization’s quality improvement, riskmanagement and patient safety program to include but not limited to NationalPatient Safety Goals, Core Measures and HCAHPS medication related initiatives.
·Completenotification reports regarding medication errors and adverse drug reactions.
·Participate in the data collection of drugutilization evaluations, adverse drug reactions, and pharmacy qualityinitiatives.
·Lead the training and mentorship ofpharmacists, pharmacy students, residents, technicians, nurses and physicians.
·Participate in the training of newpharmacists, pharmacy students, pharmacy and medical residents, technicians,nurses and physicians.
·Participatein preceptorship of pharmacy students and/or residents.
·Maintain professional competency sufficient tomeet current and future needs of the facility.
·Has knowledge of the patient population theyserve and apply that knowledge in the care of those patients.
·Provide basic technical support for automateddispensing machines and other pharmacy computer systems.
·Effectively supervise pharmacy operationactivities and provides support to the whole pharmacy team.
·Compliantwith facility pharmacy schedule to work physically at patient care areas tointeract with medical staff, nurses and other members of the healthcare team.
·Effectively and proactively assists withpharmacy and medical staff regarding drug therapy issue resolutions.
·Trainand participate with Disaster Team drills and real-event response as assigned.
·Trainand participate with Code Teams as assigned.
·Work with the Pharmacy Director andappropriate manager to identify processes to meet or exceed all controlledsubstance diversion prevention and monitoring standards.
·Comply with all regulatory standards. Work with the Pharmacy Director to identifyand develop processes to meet or exceed all regulatory standards.
·Prioritizework and lead others to comply with facility medication turn-around time policyand procedure. Identify areas for improved efficiency.
·Superviseand assist pharmacy technicians and other support staff effectively.
·Participate in establishing pharmacy goals andobjectives and assists in the implementation.
·Participate with various nursing and medicalstaff committees as necessary.
·Act as supervisor in charge as assigned.
·Participate in the continuous quality reviewor order sets and/or computer drug entries.
·Participate in the continuous qualityimprovement of pharmacy operations (operational/clinical).
·Provideinput for improving pharmacy computer system programs relating to pharmacy,nursing and physician programs.
·Practiceand adhere to the “Mission and Value Statement”.
·Performother duties as assigned.

A. Licensure/Certification/Registration: Current licensure by the Colorado Board of Pharmacy as aregistered Pharmacist. BLS and ACLS required within first 2 monthsof employment.
B. Education: B.S. orDoctorate of Pharmacy (Pharm.D.)
C. Experience: Minimumof 1 year of hospital clinical pharmacy experience or one of the following:
·At least 2 years of general hospitalpharmacy experience
·Completion of hospital PGY1/ pharmacy generalpractice residency or fellowship
·Specialty BCPS certification or Other certification (e.g. FCCP, FASHP)
